Observed unlabeled chemical spray bottle in the laundry room. 511-6-2-.17(2) - Housekeeping Carts Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Label all chemical spray bottles

Observed debris inside the dresser drawers in room 110. 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Clean inside the dresser drawers as often as necessary

Observed missing lightbulbs above the bathroom vanity in rooms 105 and 110. 511-6-2-.08(6) - Artificial Light Toilet rooms, lavatories and bathing facilities shall be provided with soap, artificial light, and hot and cold water under pressure. Replace missing lightbulbs with new lightbulbs

Observed torn A/C filter in room 101. 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Replace damaged/torn A/C filters with new A/C filters

Also, observed live bugs in room 208. 511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. Have a licensed pest control applicator come to facility as often as necessary

Observed stained sheets in room 208. Also, observed burn holes in sheets in room 202. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Replace damaged and stained sheets with fresh new clean linens

Observed debris under the bed in rooms 105 and 208. 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Clean under the bed as often as necessary

Observe debris along walkway and parking lot. 511-6-2-.18(2) - Grounds Free of Litter Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter. Clean-up litter as often as necessary

Observed the bathroom vanity missing a light bulb. 511-6-2-.08(6) - Artificial Light Toilet rooms, lavatories and bathing facilities shall be provided with soap, artificial light, and hot and cold water under pressure. Install a new lightbulb where lightbulbs are missing as often as necessary
