Not all employees have signed the employee health policy. This document is what the facility uses to train on the employee health policy. CA: Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Facility given a blank employee health policy, and has until Monday to get everyone's signature to verify they have been trained.

Invoice for key drop delivery does not have temps for items that were delivered. Agreement states driver is to temp 1 cooler and 1 freezer item. Key drop deliveries shall show the condition and temps of items delivered. CA: Temps are to be recorded upon delivery or key drop agreement may possibly be terminated by the store.

Employee using phone on line. Employee put phone away and started working with food without washing hands. Employees shall wash hands after engaging in activities that contaminate the hands; COS: Manager trained employee on when to wash hands.

Build up of black residue on ice chute in front soda machine. Food contact surfaces shall be clean to sight and touch; CA: Manager to clean ice chute.

Build up of black residue on bottom shelf of ice machine, and griddle sides. Non-food contact surfaces shall be cleaned to prevent accumulation of residues.

Floor under 3 comp sink had debris and grey build up. 3 comp sink leaking water on floor. Mop sink constantly running water. Physical facilities shall be maintained in good repair, and cleaned to prevent build up and contamination of food products. CA: 3 comp sink not filled as full to prevent leaking. Maintenance notified. Floor to be mopped.

Invoices using key drop delivery do not have temps of food during delivery. Records for key drop delivery shall show the delivery conditions and temp of the products; CA: Going to get in touch will delivery company will email me invoice with temps.

Not all employees have signed employee health policy. PIC shall demonstrate in a verifiable manner that all employees are/were informed of their responsibility to report symptoms and/or illnesses; CA: Employees to fill out form.

Multiple sauces in sauce squeeze bottles not labeled. All food in working containers taken out of their original container shall be labeled.

No inspection report on drive thru window. The most current inspection report shall be displayed on drive thru windows.

No key drop delivery agreement. The business entity providing key drop deliveries to the establishment shall certify in writing to the establishment that products delivered will be under its control throughout the delivery process to the establishment, and that all products will be delivered to the establishment during the key drop delivery hours. The food service establishment shall maintain records to the written agreement. CA: Manager is going to contact company to get the written agreement -> email within 3 days with the agreement.

No paper towels available at the handwashing sink by the 3 comp sink. Each handwashing sink shall be provided with hand drying provisions. COS: Added during inspection.

Fries, tacos (in walk in freezer) and sauce stored on the floor. Food shall be protected from contamination by storing 6" off floor.

No inspection report posted in drive thru window. The most current inspection report shall be displayed in public view at all times.

Observed drink on food rack. All employee belongings shall be stored in a designated area.
