Annual gas fired equipment inspection was not on file or available to review during inspection; CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection. Bathtub paint peeling in room 114, 210, and 212. CA: Toilet room fixtures must be kept clean and in good repair

Missing caulk on toilet in room 116. Damaged tile on jacuzzi and missing caulk on jacuzzi. Bathtub was chipped in rooms 229, 302, and 329. Bathtubs need to be re-caulked in rooms 229 and rooms 329. Residue in toilet of multiple rooms; dead bug in toilet of room 110; CA: Toilet rooms and fixtures must be kept clean and in good repair

Ventilation not working in bathroom of room 329, lobby men's restroom and lobby women's restroom; CA: Restroom shall be mechanically ventilated

In all rooms, walls must be painted, walls damaged, and windows need to be re-caulked. Water damage at top left corner of window in room 110. Mold like residue on ceiling near vent of room 228. Dead bugs in room 228. Carpet beneath pull out couches need to be vacuumed. Walls next to ice machine and floor showing signs of water damage. Walls next to ice machine peeling. Third floor maid room has mold on wall; CA: Floors, walls, ceilings, windows, and doors shall be properly maintained, and kept clean

Ice machines on each floor must be cleaned. Ice machine leaking from protrusion on back; CA: Provided machines should be maintained in good repair and properly cleaned

Back wall of laundry room behind washer damaged; CA: Laundry facilities shall be kept clean and in good repair

Chips in the tubs in rooms 104, 217, 226, 329. Vents not working in rooms 214 & 329. Toilet rooms and fixtures shall be kept in good repair. Bathing facilities shall be mechanically ventilated

Wall behind ice machine on third floor had a large gash in it. Ceiling tiles above the ice machine on the second floor damaged. Floors, Walls, Ceilings shall be in good repair

Ceiling tiles had water damage in the main laundry area. Laundry areas shall be kept in good repair

Room 108 had water damage on the nightstand beside the tub. All furniture shall be maintained in good repair

Permit was not displayed. A tourist accommodation permit shall be displayed

Ice machine on third floor had build up of black residue on ice chute. Ice equipment shall be kept in a clean place and shall be kept clean. POC: Maintenance notified, will clean the ice chutes

Clean lines were stored on the floor. Walls by the washers/dryers were corroded. Two ceiling tiles had water damage in the main laundry area. Laundry areas shall be kept in good repair and clean linens shall be stored 6" above the floor. POC) Maintenace shall fix wall and ceiling and linens will be move to at least 6" above the floor

In room 14, the bed bug cover has a stain on it. All of the bed coverings shall be maintained clean and in good condition

Toilet in room 214 needs the base re-caulked. Restroom facility fixtures shall be maintained in good repair

Ice machine on both floors 2 and 3 have black residue build up in the ice chute. Ice equipment shall be kept in a clean place and shall be kept clean. POC: Maintenance notified, will clean the ice chutes
