Observed dead roach under booster seat on the right side of the bar and behind the brown chair in room 110. As the inspector was completing the inspection of the rooms, pest control was entering to exterminate the facility

Hotel Manager was unable to find a copy of the annual gas fire equipment inspection. CA: Inspector provided PIC with a copy of the inspection form and informed her that the inspection will need to be completed within the next week

Observed the facility did not have their most current inspection posted for public view. CA: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed door that leads to first floor seating was not tight fitting. CA: All openings to the outside must be tight fitting

Observed issues notated in the last inspection in regards to conditions of furniture, doors, and walls had not been addressed. Multiple rooms had cracked light switch and plug covers, damaged lamps, chipped paint on the walls, cracked toilet bases, black substance on the ceiling and missing door knob CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed multiple housekeeping carts with spray bottles that were not labeled throughout the hotel. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed multiple room inspected had water temperatures in the bathrooms reaching greater than 120F, some temperatures were as high as 146F. CA: Hot water in all guest rooms shall not exceed 120F

Observed live roaches in the drawer of room 212 during inspection. CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed multiple rooms inspected did not have mattress covers on the beds. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed trash all around facility to include beer and water bottles, chip bags, and other trash. CA: Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter

Observed furniture, doors, and walls located in the guestroom and hallways were stained and had dirt debris on them. Observed 2nd floor storage room door no longer locked and can be accessed by guest and the light to the room no longer working. Observed electrical socket in room 335 was missing cover. Observed multiple microwaves had food debris or rust inside of them. Observed multiple TV stands were warped or had broken handles. Observed carpet on the 2nd and 3rd floor were stained. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed all rooms inspected with the exception of one did not have coverings over the mattress. CA: Washable mattress pads or covers shall be used on all mattresses

Observed orange, skittles, pet hair and food debris under and on pull out couches in guest rooms. CA: Discussed ensuring that staff pulls couches out and cleans under them after each guest

ROOM 329: Observed black mold like substance in the bathroom shower. CA: Discussed cleaning the substance with warm soapy water to ensure the substance is not on the shower wall and ceiling

Observed door that leads to first floor seating was not tight fitting. Observed first floor stairwell exit was not tight fitting. CA: All openings to the outside must be tight fitting

Observed chemicals in laundry room, housekeeping rooms and house keeping carts were not labeled with common name. CA: All chemicals must be labeled with common name at all times. PIC agreed to ensure that if labels are unavailable staff will label each bottle with a sharpie. COS: Employee wrote common name of chemical on bottle with a sharpie obtained from laundry room

Observed the following water temperatures: Room 110: 140F in bathroom sink, 118F in bathtub Room 205: 135F in bathroom sink, 118F in bathtub Room 208: 143F in bathroom sink, 129F in bathtub Room 220: 137F in bathroom sink, 124F in bathtub Room 230: 143F in bathroom sink, 119F in bathtub Room 309: 143F in bathroom sink, 125F in bathtub Room 319: 141F in bathroom sink, 138F in bathtub Room 327: 139F in bathroom sink, 128F in bathtub, 133F in bar sink

Hot water in all guest rooms shall not exceed 120F

Observed tissue paper between mattress and box spring near the wall in room 208. Observed a bottle cap between the mattress and box spring near the wall in room 220. Observed a plastic spoon between the mattress and box spring in room 309. Observed a candy wrapper left between the mattress and box spring in room 319. Observed trash and food crumbs on the sofa bed mattress in room 327. Observed some mattresses without washable mattress pads or covers.

Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, and mattress and bed coverings shall be kept clean

Observed no tourist accommodation inspection report displayed.

The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times

Observed large amount of hair in a pile on the floor near the entrance to the bathroom in room 208. Observed goldfish crackers and crumbs on the floor beneath the sofa bed in room 327. Observed the paint cracking and peeling off the wall just above the bathroom sink in room 230.

Floors, walls, ceilings, windows, doors, and all other appurtenances shall be properly maintained in good repair and shall be kept clean

Observed multiple spray bottles on housekeeping carts without their contents labeled.

Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
