Observed dead roach under booster seat on the right side of the bar and behind the brown chair in room 110. As the inspector was completing the inspection of the rooms, pest control was entering to exterminate the facility
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Hotel Manager was unable to find a copy of the annual gas fire equipment inspection. CA: Inspector provided PIC with a copy of the inspection form and informed her that the inspection will need to be completed within the next week

Observed the facility did not have their most current inspection posted for public view. CA: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed door that leads to first floor seating was not tight fitting. CA: All openings to the outside must be tight fitting
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation
Observed issues notated in the last inspection in regards to conditions of furniture, doors, and walls had not been addressed. Multiple rooms had cracked light switch and plug covers, damaged lamps, chipped paint on the walls, cracked toilet bases, black substance on the ceiling and missing door knob CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
33 - housekeeping carts and storage areas maintained; clean/sanitized items protected from contamination

Observed multiple housekeeping carts with spray bottles that were not labeled throughout the hotel. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
