Observed last annual gas fire equipment inspection was conducted on 03/17/2025. PIC stated that he would call the inspector and see if he could get him out today. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority

Observed toxic chemicals being stored on multiple housekeeping carts without a label with the common name to identify the chemical. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed facility had bar of soap being provided as hand cleanser in the women's central guest restroom. CA: Soap furnished in public wash rooms or baths shall be dispensed from approved containers designed to preclude contamination of the contents by individual contact

Observed ceiling tiles in each housekeeping/maintenance room had missing ceiling tiles and damaged metal ceiling structure. Observed a block of the ceiling had been cut open on the fourth floor between the ice machine and the stairwell door. Observed white and yellow fuzzy substance on the carpet outside of room 220. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed the ice machine on the 1st and 3rd floor had build up of a black substance on the inside of the ice dispenser. CA: Manager instructed employee to go clean both machines

Observed women's restroom was out of order. When the inspector opened the door it was observed that there was what appeared to be dried sewage and black specs on the floor around the floor drain. CA: Toilet and lavatory shall be provided at all tourist accommodations. Such facilities shall be easily accessible, convenient and available to patrons at all times. Inspector will follow up in a week to ensure that restroom is accessible

Observed 2 large dead roaches next ice machine on the 2nd floor. CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed that PIC did not have copy of the annual gas fire equipment inspection. Per discussion she explained that they believe the last one may already be expired. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority. Will follow up in a week to verify that all equipment has been inspected

Observed the following water temperatures: Room 124: bar sink 157F, bathroom sink 150F, shower 134F Room 204: bar sink 104F, bathroom sink 94F, shower 100F Room 208: bar sink 144F, bathroom sink 143F, shower 140F Room 214: bar sink 172F, bathroom sink 141F, shower 143F Room 306: bar sink 131F, bathroom sink 133F, shower 121F Room 320: bar sink 136F, bathroom sink 139F, shower 131F Room 326: bar sink 126F, bathroom sink 130F, shower 123F Room 410: bar sink 144F, bathroom sink 153F, shower 137F

Hot water in all guest rooms shall not exceed 120F

Observed ice bucket half full of water left in refrigerator in room 208. Observed container of vanilla ice cream left in the freezer in room 208. All furniture, draperies, appliances, carpets, and other accessories must be maintained clean and in good repair

Observed multiple ceiling tiles missing throughout the storage/housekeeping closet on the 4th floor. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed various chemicals stored on housekeeping carts on the 2nd and 4th floors without labels. Observed bleach smelling substance on a housekeeping cart in the laundry room unlabeled. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed multiple glasses, mugs and bowls inside of rooms 120, 125 and 414 that were visibly soiled. Observed 2 glasses in room 120 with a sticky substance on them that could be wiped away with a alcohol swab. Observed bowls and mugs in room 125 with orange food debris and coffee grounds in them. Observed dishes in room 414 that were smeared with a whitish substance as if they were wiped with a chemical rag. CA: If establishment is to utilize multi-use utensils they shall be thoroughly cleaned and sanitized after each guest

Observed pull out couches in rooms 120, 125 and 408, all had debris on them. Observed room 120 had a cap to a drink, orange toy, and food crumbs. Observed room 125 food debris on the pull out couch and ripped paper towels and fortune from a fortune cookie under it. Observed pull out couch in room 408 had what appeared to be white pet hair all over the pull out couch. CA: All furniture must be kept clean and in good repair

Observed multiple ceiling tiles missing throughout the establishment in the housekeeping closets. Observed leak on the second floor that caused a few tiles to fall. Observed trash can being used to catch dripping water. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed the lights in the laundry room were not working during inspection. Per conversation with PIC there is a suspected short and the lights have been out for almost 2 weeks. Inspector requested that the PIC contact the owner to get a eta as to when the lights will be fixed.

511-6-2-.17(1) - Laundry Facilities Where laundry facilities are provided, they shall be separate from other facilities, of sound construction, and shall be kept clean, and in good repair. Laundry rooms shall be vented to the exterior and well lighted

Observed toxic chemicals stored on the house keeping cart being utilized on the first and third, that were not labeled with their contents. Observed yellow liquid on the floor of the 3rd floor housekeeping closet that was not labeled. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
