Observed the floors in multiple rooms needed to be cleaned. Observed debris around the base of the walls and on the floor. CA: Floors, walls, ceilings, windows, and doors shall be properly maintained and kept clean

Observed there was no toilet tissue provided in the central men's restroom. CA: Toilet tissue shall be provided in a dispenser at each toilet at all times

Observed rooms 206 and 217 were not tight fitting. CA: All doors should be tight fitting to avoid the entrance or harborage of insects and rodents

Observed excessive rust on the racks of the fridge in room 206. Observed excessive rust and a burn hole in the interior of the microwave in room 221. Microwave was discarded. PIC stated that all refrigerators not in good condition are to be replaced within the next few months

Observed a leaking sink in room 200 with a puddle of water on the floor. Observed some buildup/wear and tear on several bathtubs throughout the facility. Observed a loose sink faucet in room 219. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed gummy bears on the floor next to the nightstand in room 204. Observed dust/dirt on the floors in various rooms. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Facility could not provide an inspection reports for gas fired equipment conducted withing the last 12 months. All gas water heaters and clothes dryers must be inspected and a Georgia DPH Gas-Fired Equipment inspection form should be filled out for each piece of equipment

observed refrigerators Not functioning in rooms 244 and 247. Observed dead roaches in refrigerator in room 149. Lamps in guest rooms are rusty and wobble. Observed wardrobes where present appear in poor repair
