Observed central restrooms were not being made available to guest and patrons. The doors to the establishment are being kept locked. COS: Doors to establishment unlocked. CA: Toilets, lavatory, and bathing facilities shall be provided at all tourist accommodation

Observed trash in bags in the breeze ways and on the grounds around the facility. COS: As the inspector and the PIC walked the grounds he picked up most of the trash. CA: Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter

Observed front door to the establishment was not tight fitting. CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed multiple spray bottles, located on the housekeeping cart outside of the laundry room, were not labeled with the common name of the chemical. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed the sinks in the central restroom and room 121 were not providing hot water at the time of inspection. Owner attempted turning the faucet and waiting as well to see if the water would get warm. CA: Owner states that he will fix both sinks and ensure that they are in working order

Observed central restroom was not being made available to guest and patrons. Per conversation with owner they keep the lobby locked until "around noon". Observed "out of order" sign on restroom and later discovered through conversation with the owner that the restroom is in working order there is just a sign on the door. Owner inquired if he could have the toilet removed from the restroom and was informed that central restrooms must be provided and shall have a toilet. COS: Sign removed from door and owner informed that toilet and lavatory and shall be provided at all tourist accommodations. Such facilities shall be easily accessible, convenient and available to patrons at all times

Observed multiple guest rooms had dead and live bugs. CA: Owner to contact an exterminator and have the hotel professionally treated. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed most of the linen, towels and rags were stained and being used in the guest rooms. CA: Inspector showed stained linen, towels and rags that were located in the guest rooms and laundry to the owner and informed him that if the stains could not be removed than they shall not be used in the rooms for the guest

Observed that owner was unable to provide most recent pest control records. Inquired as to when the hotel was last treated and was made aware that he was treating the hotel himself. CA: Owner informed that a record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label
