6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple TCS items being stored in RIC at temperatures above 41 F. Items incluyde pastor(48 F), Barbacoa/ beef (48F), Rice(48 F). I had PIc discard items dated from 12/1-12/9 that were out of temp, and allowed her to cool any items prepped today. PIC stated that she will have maintenance come by to take a look at unit. CA: PIC must make sure items are stored at temperatures of 41 F or below inside unit.
511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above.

PIC has not provided a water sample bacterial report for this quarter. PIC stated that she will have a report done by Monday. CA: PIC must send a bacterial water sample report quarterly and have a W33 chemical test done yearly.
511-6-1.06(1)(f)2 - Sampling Report (C) 2. Sampling Report. The most recent sample report for the non-public water system shall be retained on file in the food service establishment or the report shall be maintained as specified by the Department.
11D - thermometers provided and accurate

Observed several RICs in establishment that did not have a thermometer placed inside. COS: PIC placed new thermometers in units. CA: A TMD must be placed in all cold holding units.

A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.
15B - warewashing facilities: installed, maintained, used; test strips

Observed test strips that were expired in main kitchen. Test strip exp date was 07/2025 and today is 12/10/25. CA:PIC must buy replacement test strips as soon as possible.
