Room 110- TV stand & dresser observed with holes at top.; Room 110- Stains observed on curtains and upholstered chair.; Room 156- 1 upholstered chair observed with small tears. The other upholstered chair in room observed with cigarette burns.; Room-160 Upholstered chair observed with small tear

Room 110- Stains observed on sheets, mattress pad and mattress.; Room 156- Left bed mattress observed with stains. Right bed in room observed with stains on pillow and mattress cover. Cigarette burns observed on comforter.; Room 137- Mattress in room observed with stains.; Room 160- Matress observed with stains in room

Room 110- Observed stains in tub. In need of resurfacing.; Room 156-Observed stains in tub and in need of resurfacing. Room 108- Reattach plate around shower fixture back to wall. Tub in need of caulking where shower wall and tub meets.; Room 160- Tub Observed with stains and rust. Towel rack in room observed broken.; Room 137- Observed shower rod with staining and rust and in need of replacement. Observed broken soap dish in tub/shower combo.; Room 156- Crack observed on tile in bathroom. Tub observed with stains. Light switch observed slightly detached in bathroom. Ceiling in bathroom observed with damage indicative of water damage

Room 158- Observed chair in room with chipped/cracked surfacing. Room 156- Observed upholstered chain with cigarette burns. Room 149 & Room 158- Observed missing knobs on dresser. Room 114- Observed dresser with peeling of sealant of wood

Room 149- Observed cigarette burns in the comforter of bed and stains on pillows and pillow cases in the room. Room 158- Observed mattress protector with stains and cigarette burns. Room 156-Observed cigarette burns on top cover sheet, pillows observed with strains and mattress pad/protector of bed with stains

Observed damaged shingles with lifting above room 161 of motel. -Observed missing soffit vinyl siding above room 132

Room 156- Observed spots of mold on paint on wall in bathroom area. -Observed missing/ damaged smoke detectors in room 158 and room 114

Room 149-Observed missing wall tiles in bathroom, water damage evidence on ceiling of bathroom, Room 158- Observed hole in wall near tv stuffed with cloth/paper etc. Room 156- Observed bathroom wall tiles with cracks and chips and evidence of water damage on the ceiling of the bathroom. Spots of mold observed on paint in bathroom. Room 114- Observed cracks in bathroom wall tiles and flooring in front of room with damage to flooring tiles

Room 110- Observed live and dead roaches inside the mini fridge, a dead roach in the tub, and a dead roach on the tile in the bathroom. Observed dead roach in bathroom, dead roach in dresser drawer (containing bible)

Room 162- Observed small stains on the pillows and small tears on the mattress pad. Room 160- Observed stains on the pillow cases. Room 108- observed stains on pillow. Room 110- Observed pink stains on the box spring

Observed damaged shingles with lifting above room 161 of the motel

Room 108- Observed broken tile on flooring near bathroom door. Room 160- Wood tiles have damage indicative of water damage

Room 114- Has cigarette burn marks on comforter, sheets and mattress pads

Room 114- Has broken soap dish in tub/shower combo. Tub in bathroom needs resurfacing

Observed water damage on ceilings from leaks in rooms (150, 142, 114, 164, and room 107

Recommended continued sealing of rear window in laundry room; Still has gap present to outside near window. -Recommended sealing around window ac unit in room 142

Room 150 (Closed)- has evidence of water damage on ceiling. Room 107 (Currently used for storage)- has water damage on ceiling in bathroom area. Room 142- Has Water damage on ceiling in bathroom area; Has water damage to vinyl plank tiles in sleeping quarter area; Vinyl planks in sleep area in disrepair. Room 164 (Occupied)- Has water damage on ceiling in bathroom area

Room 125- Observed 2 dead roaches behind toilet, observed spider webs in bathroom and night stand area, observed dead roaches inside refrigerator, small roaches near wall across from refrigerator. Room 159- Observed 2 live roaches and 2 live spiders in dresser

Room 109- Has stains on curtains near window, room cabinet has flaking wood inside. Room 125- Has stains on curtains near window

Room 125- Observed stains on pillow cases, comforter, sheets and mattresses. Room 159- Observed burn mark on mattress pad, small stains on pillow case and mattress pad. Room 109- Sleeper sofa sheets are heavily stained also observed stains on mattress pad and burn marks on sheets

Room 160- Observed broken towel rack, tub in need of resurfacing, add additional tiles near tub. Room 159- Missing towel rack, tub in need of resurfacing, and leaky faucet at tub. Room 125- Tub is rusty and in need of resurfacing, missing soap dispensers and broken tiles where dispensers are missing. Room 109- Bathroom light is not working, tub is in need of resurfacing, seal hole around shower head in the bathroom, cold knob at faucet for bathroom sink is missing

Room 159- Paint is peeling on ceiling in bathroom. Room 160- Ceiling in room has evidence of warping and in bathroom

Room 109- Missing Light shield for room Room 125- Missing Light shield and 1 bulb

Room 157- Upholstered chair has stains. Room 161- Upholstered sleeper bed is peeling

Room 110- Mattress pad has stains and one of mattresses in room has small holes.; Room 157- Stains observed on mattress and mattress pad; Room 161- Stains observed on sheets and mattress pad

Room 109- Missing towel rack rod observed

Room 109- Light shield was observed missing

Room 110- Observed dead roach in bathroom and dead bugs in cabinet drawer; Room 157- Observed dead roach on head board spider and dead roach in bathroom; Room 161- Observed live and dead roaches in cabinet and drawers

Room 111- Observed cracked/ broken toilet lid; Room 156- Observed broken towel rack and cracked shower dish in bathroom; Room 111- Observed rusty soap dishes

No ventilation vents in bathrooms for any inspected room (Rooms 111, 110, 156, 157, 161)

Observed holes and openings in both water heater rooms leading to outside of building. Observed hole near window in laundry room

Room 157- observed peeling paint on bathroom ceiling, small hole in wall under ac unit in room; missing tiles in bathroom where missing towel rack is located, hole in wall near shower head
