Room 109- Observed 3 live roaches in the bathroom. Observed 1 dead roach inside the refrigerator. Room 127- Observed 1 live roach in room. Have both rooms treated

Facility unable to produce documentation for gas fired equipment. Per previous inspection(12/05/25) facility was instructed to have equipment tested by end of year

Room 104- Curtains observed with large amounts of staining and holes. Observed sofa bed and mattress in room with large staining.; Room 109- Observed broken window. Staining also observed on upholstered chair and on pillow on bed. Trash observed in night stand; Room 127- Observed cigarette burns on upholstered chair in room

Room 104- Large amount of Cigarette burns observed on comforter, sheets, and fleece blanket. Staining observed on sheets and fleece blanket.; Room 109- Pillow observed with very large stain. Mattress pad also observed with staining.; Room 127- Stains observed on mattress cover

Room 104- Ventilation vent in restroom observed non operational. Mold observed on ceiling in shower and on ceiling in bathroom; Room 127- Observed mold on ceiling in the bathroom

Room 104, 109 and 127 -Observed with baseboards and corners of floors with accumulations of dust and debris

Room 102 & 125- Night stands observed in room without drawers. Room 102- Observed stained and worn curtains. Observed upholstered chair with stains on cushions. Room 103- Upholstered chair observed with multiple cigarette burn marks. Curtains observed with very large stains

Room 102- Observed cigarette burns on mattress comforter and sheets. Observed stains on sheets in room. Room 103- Numerous cigarette burns were observed on sheets, comforter, and mattress pad. Stains were observed on sheets and comforter in room. Observed thin comforter under large comforter with large bleach stains. -Observed cigarette burns on sheets and comforter blankets stored in clean linen room. Towels and wash cloths were also observed with staining in clean linen room

Observed trash, old food containers, empty beer can and alcoholic bottles on grounds of hotel

Room 125- Observed ventilation vent not operating. Small amount of dust and mold debris were observed on ceiling near vent and shower. -Room 103- Observed ventilation vent not operating

Room 103- Front door observed damaged with filled holes and back of the door additional damage of cut/scrape marks. Room 103, 102 &125- Observed baseboards and corner of floors with accumulations of dust and debris

Room 125- Observed bed sheets on mattress with staining. Also observed pillow on bed with mildew staining. Sofa bed in room had staining on mattress and crumbs and debris inside

Room-Observed tile underneath sink in the bathroom with rusty staining from leak from hot and cold water cut off values

Room 125- Observed ventilation vent not operating. Accumulations of dust and debris were also present

Observed window curtains in room 103 and 106 with strains. *Replace window curtains

Room 107- observed broken blinds at windows; Room 116- Observed holes in curtains

Room 107- Stains observed on mattress protector, comforter and sheets; Room 116- Stains observed on mattress pad, on upholstered chair observed stains and tearing, also observed burn holes in sheets; Room 119- Observed burn holes on comforter, stains on mattress pad

Room 119- Tub/Shower combo in need of resurfacing and cleaning;

Room 107- Mold present on ceiling and wall in shower; Room 116- Mold on ceiling in shower, vent in bathroom not operational

Room 119- Door has soot/staining on door; Room 107, 116, and 119- need general cleaning of baseboards, cracks and crevices in corners; Room 119- trash and debris observed behind dresser. Sleeper sofa observed with dirt/debris inside. Baseboard behind refrigerator also has damage and needs repair

Room 119- upholstery chair is in need of replacing due to deteriorating conditions and staining; Room 103- Microwave is rusting, folded chair in room is broken.; Room 112- Needs sleeper sofa and mattress in sleeper sofa replaced due to being badly stained. Room 101- Mini fridge in need of cleaning inside fridge portion and freezer portion. Room 119, 101, 103, and 112 all in need of cleaning of baseboards due to accumulation of dirt and debris

Room 119- Sheets, mattresses and mattress pad all stained and need replacing. Mattress pad and sheets on one bed have cigarette burns; Room 101- Stains on matress and mattress pad in room.; Room 103- small stains on mattress of bed and mattress pads, pillow cases in need of replacement.; Room 112- Sleeper sofa and matress pad in need to be replaced due to heavy straining. Pillow cases all in need of replacement

Facility does not have current inspection report posted. Currently posted inspection report is dated 12/27/22

Room 119- Tub/shower combo in need of resurfacing and cleaning due to being badly stained.; Room 101- needs cover added to shower drain in shower

Room 119- Closet light bulb needs replacing

Room 127- Observed live roaches in sleeper sofa; Observed rat droppings in laundry room

Room 127- Rusty Microwave needs replacing; Dirt dust and debris present in all rooms on base boards around room

Room 112- Heavily stained mattress sofa bed, cigarette burns on mattress covers; Room 124 Stains on mattress covers; Room 128- Stains on mattress covers

Room 124- mold is present in shower on ceiling and on missing tiles in shower; Room 127- Mold is present in shower on ceiling and on missing shower tiles; Room- 128- Mold is present in shower on missing shower tiles. Room 112- Needs vent cover; Room 124- Ventilation system in bathroom is not operational; Room 127- Ventilation system in bathroom is not operational
