All cold held items were in compliance. No hot holding was observed. All food in freezers frozen solid. An unscored informal will be conducted on or before August 22nd, 2025 to ensure that a food safety manager certificate has been posted that is not posted at another food service establishment and to ensure that the employee health agreements forms have been signed or documentation has been provided that employees have been made aware of what illnesses and symptoms to report to the Person in Charge. 2-2A: .03(2)(m)When asked for signed employee health policy forms, PIC could not provide EH with any signed documents for any employees at the facility. PIC also could not demonstrate in any other verifiable manner that food employees had been informed of their responsibility to report symptoms and/or illnesses. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. 2-2A: .03(1)(c)2,3,17Person in Charge (PIC) was not aware of what symptoms and diseases to report per the employee health policy. Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissible through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. (Pf) Corrective Actions: EH educated PIC on what symptoms and illnesses needed to be reported based on the employee health policy.
