Observed magnetic knife holder and "blue" cutting board not clean to sight. Clean and sanitize items. COS. Observed significant accumulations of scale/mineral deposits on interior of dish machine. Clean dish machine at a frequency to prevent accumulations. Complete by 9/7/25.

Observed numerous bowls, plates, storage containers and chaffing pans stored upright and exposed. Observed clean utensils in store room stored with food ends up and exposed. Store items inverted or covered. COS.

Observed missing hood filter on left end of hood. Replace filter by 9/7/25

Observed non-food contact surfaces (sire rack shelves especially in WIC, cook line equipment sides, tops and utility lines, and interiors of reach in units) with accumulations of debris, food particles and grease. Clean surfaces at a frequency to prevent accumulations by 9/7/25.
