Did not observed CSFM certification in facility. CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM)

Observed employee preparing pizza on make line while wearing watch on wrist. CA: No jewelry on hands or wrist allowed while preparing/handling food, with the exception of a plain wedding band. COS: Employee removed watch & washed hands.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(f) - fingernails (pf)
Observed food employee preparing pizza on make line without gloves on with artificial nails. CA: Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Pf Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. COS: PIC instructed employee to wear nails.
14B - utensils, equipment and linens: properly stored, dried, handled

Observed multiple containers in clean dishwashing area wet stacked. Air-Drying Required. CA: After cleaning and sanitizing, equipment and utensils should be air-dried or used after adequate draining before contact with food. COS: PIC to rewash and restack dishes.
