4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed opened container of raw chicken stored touching open container of cooked beef, and opened container of raw chicken tenders stored above cooked lasagna which was stored directly on opened container of raw chicken wings in lower unit of prep top cooler. CA: Food to be stored by internal cook temperature. COS: PIC rearranged cooler to separate raw from RTE food items.
6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed TCS food items holding at a temperature greater than 41F in reach in cooler in front food service area. CA: Cold TCS food items to be maintained at 41F or below. COS: PIC stated food had been in cooler since day before therefore discarded. Internal thermometer reading at 55F. PIC notified to not use unit for TCS food items.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(b) - working containers, common name (pf)
Observed spray bottle containing liquid stored on shelf above single-service items. Spray bottle did not have label. CA: Working containers taken from bulk supplies shall have common name and be stored in designated area to prevent possible contamination. COS: PIC labeled bottle as sanitizer and moved to designated area.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.07(3)(d) - handwashing signage (c)
Observed no handwashing signage in restroom. CA: A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed multiple clean dough pans stored directly on top of apron on floor in back of kitchen. CA: cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored in a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least 6 inches (15 centimeters) above the floor. COS: PIC moved dough pans to be re-cleaned and sanitized and stored properly.
17C - physical facilities installed, maintained, and clean

Observed walls in restroom covered in fake grass (not smooth or easily cleanable). CA: Walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Observed raw wood flooring beneath kitchen equipment. Anti-slip covering was not in good repair. CA: Floors and floor coverings shall be designed, constructed, and installed so they are smooth and easily cleanable.
