Observed opened bag of raw chicken breast stored directly touching precooked ready to eat chicken and directly above raw beef. CA: Food shall be protected from cross contamination by separating raw animal foods during storage from raw ready-to-eat food and cooked ready-to-eat food; and by separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage by arranging each type of food in equipment so that cross contamination of one type with another is prevented. COS: PIC stated that chicken breast shouldn't have been there and choose to discard it.

Observed open box of raw shell eggs stored directly touching pasteurized liquid eggs and pasteurized liquid egg whites in walk in cooler. CA: Food shall be protected from cross contamination by separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage by arranging each type of food in equipment so that cross contamination of one type with another is prevented. COS: PIC moved pasteurized liquid eggs and pasteurized liquid egg whites so that they were no longer directly touching the raw shell eggs.

Observed employee handle sanitizer cloth with gloved hands then directly handle ready to eat food (bread) without removing gloves, washing hands, and donning new gloves first. CA: If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. COS: PIC had employee remove gloves, wash hands and don new gloves before handling food.
6-1D - time as a public health control: procedures and records

Observed no procedures available for time only as a public health control for pancake batter in facility. CA: If time without temperature control is used as the public health control for a working supply of time/temperature control for safety food before cooking written procedures shall be prepared in advance, maintained in the food service establishment, and made available to the regulatory authority upon request. PIC will have written procedures available by correct by date.
2-2E - response procedures for vomiting & diarrheal events
Violation of Code: [511-6-1.03(6)] Observed no EPA registered disinfectant effective against Norovirus available on-site at the food service establishment. CA: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. Observed a opening between backdoor and door frame. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: filling or closing holes and other gaps along floors, walls, and ceilings; closed, tight-fitting windows; and solid, self-closing, tight-fitting doors. PIC will have opening sealed by correct by date.

Observed no notification to customers of the presence of allergens in unpackaged food items served or sold. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. PIC will have consumer notification posted by correct by date.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed employee handling food at grill while wearing a bracelet. CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: PIC had employee remove bracelet, remove gloves and wash hands.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed server scooping grits into a bowl without wearing proper hair restraints. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: PIC removed server from food handling position.
17B - garbage/refuse properly disposed; facilities maintained

Observed no drain plug on dumpster damaged. CA: Drains in receptacles and waste handling units for refuse, recyclables, and returnable shall have drain plugs in place. PIC will have drain plug in place by correct by date.

Observed no food service permit posted in facility. CA: Post the permit in a location in the food service establishment that is conspicuous to consumers. PIC will have permit posted by correct by date.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(1)(i) - nonfood-contact surfaces (c)
Observed shelving in walk in cooler rusted. CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. PIC will have shelving cleaned and re epoxied.
