Observed shrimp, salmon, and crab in one sushi car cooler at 48 degrees F. C/A - have unit serviced so it is working properly and toss out all out of temp food. COS - chef at sushi bar tossed out out of temp food.
4-2B - food-contact surfaces: cleaned & sanitized

Observed spray cleaner for non-food surfaces being used on tables.
C/A - dilute sanitizer to food safe levels or use sani buckets and rags to wipe off tables. Return in one week to check compliance.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(e) - backflow prevention device, design standard (p)
Observed hose in kitchen attached directly to wall WITHOUT vacuum break. C/A - obtain vacuum break and install to prevent backflow of dirty water into water pipes and contaminating them.

Observed no test strips for sanitizer at time of inspection.
C/A - have sanitizer test strips for testing sanitizer.

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed damaged floor tiles on kitchen floor. C/A - repair floor to be smooth and easily cleanable.

Regulation: 511-6-1.07(3)(h) - dressing areas and lockers, designated and provided (c)
Observed employee cup drying with dishes. C/A - have employees keep their items in a separate, designated area away from customer dishes.
