Observed employee enter kitchen, put on gloves, and begin to prep food without washing hands. COS- employee washed hands C/A: Always wash hands when entering the kitchen and when switching tasks.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(4)(a)(b)(c)(e)(f) - responsibility of permit holder, pic, and conditional employees (p), (pf)
Observed no evidence of Employee Health Reporting Agreements for all employees. COS- employees signed C/A: Have each food prep employee read and sign health reporting agreements. Have the forms on file at all times.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed missing handwashing signage at handwashing sink. C/A: Obtain and post.
12B - personal cleanliness

Observed employee wearing a watch during active food prep. C/A: Only a plain band can be worn during active food prep.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed open drinks stored underneath prep table. C/A: Place employees' belongings in a designated area. Drinks must have a lid and straw.
