Cold-Holding was assessed and incompliance unless noted in the temperature log. Hot-Holding was assessed and incompliance unless note in the temperature log. Ensure back exit door self closes all the way without any assistant and without a gap. Violation will be marked accordingy on next routine inspection. If back exit door is left open for delivery, an air vent, mesh, or any protective method to keep insects out is installed. If chicken wings are left out on fryer basket until service, wings shall be 135F or above or placed on time control. Violation will be marked out accodingly on next routine inspection. Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection. Repeat violations may result in permit suspension. 2-1B: .03(5)(c)Observed food employee come into the kitchen from the serving area, put on gloves and started to handle fries. Employees must wash hands and exposed portions of arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service articles, and: 1) after touching exposed body parts; 2) after using the toilet room; 3) After caring for or handling service animals or aquatic animals; 4)after coughing, sneezing, using tobacco, eating, or drinking; 5) after handling soiled equipment or utensils; 6) during food preparation, as often as needed to remove soil and contamination and to prevent cross contamination when changing tasks; 7) when switching between working with raw food and working with ready-to-eat food; 8) before putting on gloves; 9) after engaging in other activities that contaminate hands; 10) a second time when entering the kitchen from using the restroom. (P) Corrective Actions: PIC (person in charge) had food employee removed his gloves and wash his hands.

4-1A: .04(4)(n)1Observed PIC was wearing clear gloves on top of black gloves. While wearing both gloves, PIC touched raw philly steak, discarded the clear glove and went to touch bread. If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. (P) Corrective Actions: PIC discarded his black gloves and washed his hands.
