2-2A - management knowledge, responsibilities, reporting
Observed the PIC could not provide documentation or otherwise satisfactorily demonstrate that all food employees and conditional employees had been informed of their responsibility to report health information related to transmissible diseases. A new employee had already begun food preparation duties without reviewing or signing the Employee Health Policy. CA: Food employees and conditional employees must be informed in a verifiable manner of their responsibility to report symptoms, diagnoses, and exposures associated with foodborne illness. COS: Employee reviewed the Employee Health Policy requirements and signed the documentation during the inspection.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employees preparing food without wearing hair restraints or beard guards. CA: Employees handling or preparing food must wear effective hair restraints, including beard guards for facial hair longer than one-half inch, to prevent contamination of exposed food and food-contact surfaces. COS: Employees donned appropriate hair restraints and beard guards during the inspection.
