6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed potentially hazardous food (lettuce and sliced tomatoes) cold held at greater than 41 degrees Fahrenheit while on ice bath. CA: All TCS foods that are cold held must be held at 40F or below at all times. CO: Items were discarded.
6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed hamburgers being hot held in a crock pot with temperatures below 135F. CA: All TCS foods that are hot held must be held to 135F or hotter at all times. CO: Hamburgers were placed on the grill and reheated to a temperature of 187F then placed back in crock pot and the breaker turned back on that had tripped earlier.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed food being stored in crock pots and on ice baths that had no overhead protection against cross contamination. CA: All foods must be stored covered at all times to prevent cross contamination. Co: Items were corrected on site.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed the inside of the microwave with dried food accumulation or rust. CA: All equipment shall be kept clean of accumulation that come in contact with food at all times. CO: EHS will recheck for compliance within 24 hrs.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed a large hole at the bottom of the outside entry door that has not been repaired prior to repeated violations. CA: All outer openings shall be protected against entry of pest or vermin at all times. CO: EHS will recheck for compliance in 24 hrs.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed pipe for grey water that was broken from the main drainage system and allowing grey water to accumulate on the ground.CA: All plumbing systems must be maintained in proper working conditions and in good repair to plumbing code. CO: EHS will recheck for compliance within 24 hrs.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.07(3)(d) - handwashing signage (c)
Observed no handwashing sign provided at a hand sink used by food employees. CA: A handwash signage must be posted by the hand sink notify employees how to proper wash their hands. CO: EHS will recheck for compliance within 24 hrs.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed grease accumulation on the floors under the equipment, on the equipment and on the floors in the storage room. Observed dried food debris on the walls inside the unit. CA: Nonfood contact surfaces of equipment and other nonfood surfaces shall be kept free of accumulation of food, debris or residue. CO: EHS will recheck for compliance within 24 hrs.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the step into the unit at the door in need of repair or replace. Observed bottom of door into the unit that needs to be repaired or replaced. Observed floors inside the storage facility that needs repair and recovered. The deck for the outside building needs to be repaired. The litter outside the building and grass needs to be maintained. CA: All physical features shall be maintained in good repair. CO: EHS will recheck within 24 hrs for compliance.
