Observed old charcoal grill outside room 118 in the walkway. All walkways, porches, and hallways shall be maintained in good repair. Only articles necessary to the operation and maintenance of the establishment shall be stored on the premises. Remove grills from walkway of facility

Observed gap in plumbing/wall connection for toilet water line in room 108. Observed debris on ceiling tile in bathroom in room 109. Observed old outlet exposing insulation in room 109. Observed coving coming off wall in room 109. Observed debris and dead insects behind bed in room 109. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Seal gaps in wall at plumbing conduits. Seal gaps in old outlets. Repair wall/floor coving. Replace ceiling tiles as necessary. Clean floors behind beds and furniture as often as necessary
