Observed heavy food splatter accumulated on ceilings of all 3 microwaves/mini oven warmers. Ensure food contact surfaces are cleaned and sanitized at a rate to prevent buildup.

Observed boxes of food items stored under leak in WIF with deposits of ice/water on boxes. Ensure no food items are stored under leak until repaired as this could be a potential contamination risk.

Observed food splatter on ceiling of stove. *Clean stove.

Observed build up of food splatter on microwave ceiling. Food contact surfaces must be cleaned and sanitized at a rate to prevent buildup.

Observed gap at rear exterior door where sweep is busted/missing. Door doesn't fully seal when closed. All exterior doors must fully seal to prevent pest entry.

Observed numerous flying insects/fruit flies throughout facility. Areas must be free of pests.

Observed dead roach in main kitchen. Remove dead insects.

Observed signs of water damage on numerous ceiling tiles throughout dining area. Repair leaks and replace damaged tiles.

Observed excessive dust buildup on vents in restrooms. Also noted vent cover in women's restroom to be detached/loose. Re-secure and clean vents. Also noted lack of ventilation in both restrooms (women's and men's). Repair to ensure proper ventilation.

Observed ceiling of microwave covered in food splatter and underside of drink machine/ice chute soiled. *Clean microwave and drink machine.

Observed bottles of milk that expired on 1/29/24. *Discard milk.

Observed sliced lettuce and tomatoes in walk in cooler with no form of date marking. Items must be properly date marked and used/disposed according to date.

Observed wet mop stored head down in mop sink. Mops must be stored hanging to dry.
