4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed mold growing on ice and water dispensing machine in lobby. Equipment food-contact surfaces and utensils shall be cleaned In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment:
(I) At a frequency specified by the manufacturer; or
(II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.
6-2 - proper date marking and disposition

Observed cream cheese frosting w/ use by date of 9/27 still on make line. Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition.
- A food that requires datemarking shall be discarded if it:
(i) Exceeds 7 days, not including the time that the product is frozen; P
Food item was given to PIC and then discarded.
10D - food properly labeled; original container

Observed condiment bottles with no labels.
Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.
18 - insects, rodents, and animals not present

Observed approx. 1/2 inch gap under back door (enough to let light through). Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed no inspection report at the drive-thru. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(n) - covering receptacles (c)
Observed dumpsters with doors opened. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered:
- Inside the food service establishment if the receptacles and units:
(i) Contain food residue and are not in continuous use; or
(ii) After they are filled; and
- With tight-fitting lids or doors if kept outside the food service establishment.
