Intermediate - Handwash sink used for purposes other than handwashing. Observed ice dumped in hand wash sink at cook line

Basic - Dumpster rusted out on bottom. Observed dumpster rusting out at the bottom and not able to contain items. Also Observed missing drain plug

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed multiple cups on the clean ware rack that were not properly air dried - wet nesting. Repeat Violation

Basic - Grease receptacle/dumpster not on proper pad/nonabsorbent surface. Observed grease receptacle placed on soil with no non-absorbent surface.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed spoon handle in contact with cheese at front counter. Manager removed spoon. Corrected On-Site

Basic - Open dumpster lid. Observed dumpster lid open behind establishment.

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed multiple containers stacked in clean ware rack still wet - wet nesting. Manager removed and placed items to be washed Corrected On-Site

Basic - Floor tiles missing and/or in disrepair and/or in disrepair. Observed broken tile holding water in front of dish machine

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on ceiling and fan cover in Walk-in Freezer

Basic - Missing drain plug at dumpster. Observed dumpster in cage missing a drain plug.

High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Employee cracked eggs and touched clean dishes without washing hands or changing gloves.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Item in reach-in cooler across from grill: potatoes (52°F - Cold Holding). Manager stated item placed in grill one hour prior to temperature being taken. Manager placed item in walk-in freezer to reduce temperature to 41°F. Corrective Action Taken Repeat Violation Admin Complaint

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Item in warmer above grill: fries (83°F - Hot Holding). Manager stated item placed in warmer 30 minutes prior to temperature being taken. Manager voluntarily discarded item. Corrective Action Taken Repeat Violation Admin Complaint

Basic - Garbage can located outside has no lid or lid open/broken. Lid missing from dumpster.

Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Spoon stored on cook line in 78°F water.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Legs to lettuce cutter soiled with old food debris.

Basic - Old labels stuck to food containers after cleaning. Containers stored on rack next to ice machine.
