Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed potato wedger with built up food debris on it.

Basic - Floor area(s) covered with standing water. Observed standing water pooling on the floor behind the rotisserie oven where the tiles had once been. Observed water between the tiles all along the back wall behind the rotisseries where tile grout has come up.

Basic - Floor soiled/has accumulation of debris. Observed in the prep area behind the prep table with the microwave as well as behind the thermalizer.

Basic - Hole in or other damage to wall. Observed behind the rotisserie oven a hole in the wall on the wall across from steam units in carving station.

Basic - Light not functioning. Observed the lights in the oven hood closest to the ice machine were both out.
