Basic - Old labels stuck to food containers after cleaning. On clean dish shelf in ware washing area.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Chocolate Milk and 2% Milk Containers in drive thru reach-in cooler at 47F, operator stated items had been in cooler or less than 4 hours. Operator moved some items to walk-in cooler and iced remaining product down. Re-temp at 43F at 2:53 PM. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed press soiled with an accumulation of food and grease and the fryer baskets in fryer to the left of the press. Ice machine chute and dispensing tool where cups are placed to dispense ice soiled with an accumulation of slime. Ice chute in Coke Freestyle machine soiled with slime.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine.

Basic - Working containers of food removed from original container not identified by common name. Observed rolling bin of flour without label to the right of the chicken breading station.

Basic - Missing drain plug at dumpster. Drain plug at dumpster is broken.

Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer bucket at 100ppm. Manager replaced and fixed to 200ppm. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Walk in cooler shelves.

Basic - Open dumpster lid.
