High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter installed, required vacuum breaker on end of splitter that is not attached to the chemical dispensing device.

Basic - Accumulation of black/green mold-like substance in the interior Ice chute on the soda machine.

Basic - Floor soiled/has accumulation of debris. Floor and floor drain under. Soda machine.

Basic - Wet wiping cloth not stored in sanitizing solution between uses.
