Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed water stained ceiling tiles in the pizza making area.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the area on the exterior of the soda machine spout being soiled with soda debris. Observed the top of the dough pressing machine being soiled with food debris build up. Repeat Violation

Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed the interior of the reach in coolers in the front service area being soiled with food debris.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed stained cutting boards on the pizza making line and above the rack used to store can goods.

Basic - Ceiling/ceiling tiles/vents/walls and floors soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust build up on the wall and ceiling at the three compartment sink area. Observed food debris build up on the floor under the soda bib rack.

Basic - Cutting board has cut marks and is no longer cleanable.

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed wet stacked containers on the dish drying rack. Discussed proper drying methods with manager.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food debris build up on top of the pizza dough press near the pizza line. Observed the ice shoot to the self serve fountain drink station being soiled with a black mold-like substance.

High Priority - Dented/rusted cans present. See stop sale. Observed one can of roasted red pepper strips dented at seam. Manager removed. Corrective Action Taken

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed a small amount of ice build up inside walk in freezer.

Basic - No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign in pizza box area at hand sink.

Basic - Standing water in bottom of reach-in-cooler.

Intermediate - Handwash sink not accessible for employee use at all times. Observed rolling rack in front of hand sink behind pizza oven.

High Priority - Non-food grade paper/paper towel used as liner for food container. Observed basil stored between non food grade paper towels. Employee removed the towels from the container of basil. Manager could not find documentation for paper towels to be food grade material. Corrected On-Site

Basic - Open dumpster lid. Observed dumpster lid open. Manager closed the lids on the dumpster. Corrected On-Site

Basic - Unwashed fruits/vegetables stored over ready-to-eat food. Observed unwashed tomatoes stored on top of the ready to eat salad mix in the walk-in cooler. Repeat Violation

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed the mop in the mop bucket. Manager hung mop up to dry. Corrected On-Site
