Observed Triple Sink (Quaternary 400 plus ppm). Manager diluted the solution with water and last reading Triple Sink (Quaternary 300ppm). Corrected On-Site

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hand wash sink without hot water, water at 80F located at drive through, ran water for approximately 3 minutes.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed one hole over ice machine, one hole near drive thru window and another hole near dry storage area. Also observed ceiling tiles with water damage at dining room area.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles with spider webs at dry storage area. Observed sprinkler heavily soiled with accumulated grease over conveyor oven. Repeat Violation

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed at storage shelves next to were washing area.

Basic - Floor area(s) covered with standing water. Observed standing water behind soda machine and reach in cooler at drive through.

Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed in the walk-in cooler.

Basic - Covered waste receptacle not provided in women's bathroom. Observed trash can uncovered inside stool at women bathroom.
