Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Observed at ice cream chest freezer located in back of main kitchen.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed in several reach in freezers in kitchen.

Basic - Light shield damaged/in disrepair. Observed light shield missing in mop room. Observed where dish machine is above triple sink; damaged.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles and vents soiled with dust throughout rear kitchen area.

Basic - Floor soiled/has accumulation of debris. Observed soiled behind storage units and equipment.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed at chest reach in freezer units.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter with not Vacuum breaker at mop sink. Repeat Violation

Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed at RIC from counter. Manager placed a new thermometer. Corrected On-Site Repeat Violation

Basic - Wet mop not stored in a manner to allow the mop to dry.. manager removed bathe mop Corrected On-Site

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Basic - No Heimlich maneuver/choking sign posted. Choking poster was provided. Corrective Action Taken

Basic - No conspicuously located ambient air temperature thermometer in holding unit. Front counter.

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Operator removed the gloves and cleaned his hands. Corrected On-Site

Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed clean plastic bowls by the triple sink not inverted or protected.

Basic - In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed the cutting knife of the pizza stored over the surface of the preparation area.

Basic - Single-service articles improperly stored. Observed a box of pop corn containers stored on the floor of the front counter

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed at the front counter hand wash sink no hot water. Allowed the water to run for at least Three minutes. Water temperature 79F

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed in the mop sink a splitter, the hose connection section does not have a vacuum breaker
