Basic - Worn, torn and/or soiled floors/carpeting. Observed torn floor inside of MFDV and rusted wall under the BBQ .

Intermediate - Spray bottle containing toxic substance not labeled. Observed plastic spray bottle of cleaner not labeled.

Basic - Food stored on floor. Observed container of flan stored on floor at walk in cooler. Repeat Violation

Basic - Food stored on floor. Observed containers of raw beef and juice stored on the floor at the walk in cooler.

Basic - Hotel and Restaurant license decal not displayed on mobile food dispensing vehicle.
