High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed a gallon of milk an a quarter container with lime wedges stored inside ice machine near the front counter.

Observed shrimp at 46F - Cold Holding, beef - 47F Cold Holding at reach in cooler across cook line. As per operator was put on the line at 11:30 am. Operator placed ice bath for both items. Corrective Action Taken

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker in a splitter added to the mop sink.
