Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed shelves and bottom interior or the refrigerated make table with debris buildup.

Intermediate - Handwash sink used for purposes other than handwashing. Observed the hand washing sink being used to wash the personal dishes of the operator. Operator removed the dishes to the triple sink for washing. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food debris spills on the doors and frames of the refrigerated make table and on the walk-in refrigerator shelving. Observed debris buildup on the top shelf over the make table and the bottom shelf of the preparation table.

Basic - Reuse of single-service or single-use articles. Observed food box lids reused for draining fried items. Discussed that a clean surface must be used as boxes are for the purpose of shipping but not reuse for other purposes thereafter.

High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed egg rolls (124°F); sweet and sour chicken (63°F) out on the counter for "busy time" and are not tagged with the time removed from temperature control. Operator indicates the items were placed at room temperature at approximately 12:30. The time was placed onto the containers to track use within 4 hours. Corrective Action Taken

Intermediate - Handwash sink not accessible for employee use at all times. Observed a large garbage container and large container of msg stored in front of the hand washing sink by the dish washing area and a large tub in front of the back hand washing sink. Items moved to make hand washing sinks accessible. Corrected On-Site

Basic - Bowl or other container with no handle used to dispense food. Observed bowls without handles used as scoops.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food and debris buildup on the bottom interior of the upright freezer, blackened buildup on the doors and frame of the refrigerated make table, and food buildup on the exterior of ingredients containers. Observed grease and dust buildup on the top shelves of equipment.
