Basic - Accumulation of black/green mold-like substance in the interior of the ice machine. White interior plastic cover.

Basic - Cardboard used to line food-contact shelves. Shelf next to the managers office/ back door.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on the reach in cooler doors at the pizza station.

Basic - Open dumpster lid.

Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Wall next to the assembled pizza boxes is covered with cardboard taped to the wall. Wall in disrepai next to the salad reach in cooler.

Intermediate - Handwash sink not accessible for employee use at all times. Container stored inside the hand washing sink in the dishwasher area.

Basic - Floor tiles missing and/or in disrepair. Bottom of the wall between the 3 compartment sink and the hand washing sink. Ceiling tile with water damage and mold- like substance at the front counter in the lobby.

Basic - Food stored on floor. Pizza boxes stored on the floor in the dough prep / dishwasher area.

Exit door locked. For reporting purposes only. Broken in the lobby to the right as you exit the lobby.

Intermediate - Handwash sink not accessible for employee use at all times. Observed hand sink at dough prep station blocked by trash can and has a cambro stored over faucet at hand sink. Manager removed and cleared area Corrected On-Site Repeat Violation

Basic - Ceiling tile shows water damage in front lobby

Basic - Cove molding at floor/wall juncture broken/missing. On dish area to right side of three compartment sink. Also molding missing by front counter. Repeat Violation

Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Cooked pasta, cooked beef and cooked chicken in portion bags in walk in not dated

Basic - Operator does not permit customers to use women's bathroom. Discussed with manager that restroom needs to be accessible to customers as long as lobby is open. Manager will designate one restroom for customers and one for employees
