Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employees bag on top of the reach in freezer.

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Person in charge unable to provide written procedure, emailed to operator during inspection. Corrective Action Taken

Basic - Floor tiles missing and/or in disrepair. Tile in front area with pipe that drains from hand wash sink missing piece, standing water observed in space where tile is missing.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing vacuum breaker from hose bibb in kitchen, and on right side of building.

Intermediate - Handwash sink used for purposes other than handwashing. Equipment stored in hand wash sink in front area, person in charge removed. Corrected On-Site

Basic - Covered waste receptacle not provided in women's bathroom. No covered waste receptacle, also spring missing from door, no longer self closing.

Basic - Drain cover(s) missing. In employee restroom.

Basic - Outer openings not protected with self-closing doors. Self closing mechanism missing from back door.
