At time of inspection, the facility does not have records on-file for the gas-fired equipment inspections. PIC states they [inspections] were performed in July of 2025, but does not know where the records are. Locate forms and keep on-file OR have the inspections performed again (the inspections will be due July 2026, even if the records are located)

At time of inspection, observed stained mattresses in several inspected rooms. Clean/replace stained mattresses

At time of inspection, observed two bathroom exhaust fans not functioning. Repair/replace fans, to ensure adequate ventilation

At time of inspection, observed cracked and loose tile in the doorway between the bathroom and the room. One tile moved when stepped on. Replaced cracked tiles to ensure level, secure surface

At time of inspection, observed lack of anti-slip texture/applique/mat in the tub in Room 212. Install texture/applique/mat in tub to create anti-slip surface

At time of inspection, observed stained pillows and pillow protectors in rooms 123 and 125. COS: PIC removed the stained items

At time of inspection, observed several exhaust fans in bathrooms not functioning. Repair/replace exhaust fans, to provided proper ventilation

At time of inspection, observed presence of mold-like growth in the caulking inside the shower threshold. Clean often enough to prevent mold-like growth

At time of inspection, observed cracked tiles in the bathroom; the tiles move slightly when they are stepped on. Replace cracked tiles, to ensure solid flooring surface

At time of inspection, observed lack of anti-slip texture/applique/mat in the tub in Room 201. PIC states she will have anti-slip strips added to the bottom of the tub, to prevent slipping hazard

At time of inspection, facility has not had annual gas-fired equipment inspections of the dryers and water heaters. PIC states she will have the inspections performed and will email the completed inspection forms to EH office

At time of inspection, observed stained pillow protectors in Room 213. Replace stained linens

At time of inspection, observed several exhaust fans in bathrooms not functioning. Repair/replace exhaust fans, to provided proper ventilation

At time of inspection, observed a dead roach in the kitchenette area of Room 213; also observed a dead bug on the fitted sheet in Room 213. POC: PIC states pest control company will assess room for points of entry of pests

At time of inspection, observed stained mattresses, mattress pads, and pillows in all rooms inspected (except for Room 207). PIC states all stained items will be cleaned or replaced

At time of inspection, observed mildew-like / mold-like buildup on the ceiling in the bathroom of Room 101. Also observed one exhaust fan not functioning in Room 325. PIC states the mildew-like / mold-like buildup will be properly cleaned in Room 101 and that the exhaust fan will be assessed, to ensure that it is adequately removing excess moisture from the room. PIC states the exhaust fan in Room 325 will be repaired/replaced

At time of inspection, observed lack of anti-slip texture/mats in the tubs in rooms inspected. PIC states the tubs will either be made non-slip with an applique or with non-slip mats

At the time of inspection observed worn drawers and carpet in all rooms inspected, most notable in room 214 with ripped carpet; Advised PIC to repair and replace as necessary

At the time of inspection observed leaking faucet in room 329; Advised PIC to repair and replace as necessary
