At time of inspection, observed furniture and appliances in need of repair/replacement in all rooms. Advised PIC to repair/replace dressers, microwaves, and mini-fridges

At time of inspection, observed vents in rooms 206 and 219 in disrepair. //Advised PIC to replace/repair

At time of inspection, the facility does not have records of inspections for the gas-fired equipment (dryers and water heaters). POC: Hire a company to perform gas-fired equipment inspections on all gas-fired equipment in facility. Provided PIC with a copy of the gas-fired equipment inspection form (one form must be completed for each piece of equipment).

9/8/2025: PIC states Dekle Plumbing was contacted to schedule gas-fired equipment inspections. According to PIC, Dekle Plumbing is booked for this week, but they have scheduled to perform the inspections on 9/15/2025. PIC states she will email the completed gas-fired equipment inspection forms, once the inspections have been performed.

9/17/2025: The Person-in-Charge (PIC) emailed me the completed gas-fired equipment inspection forms. The forms have been printed and added to the facility's physical file. This violation has been corrected

At time of inspection, observed stained pillow protectors in all rooms inspected. Also observed stained mattress protectors in Room 224 and Room 121. PIC took note of all stained items and states they will all be replaced today

At time of inspection, observed debris accumulated on the floor behind the nightstands in rooms 113 and 121. PIC states the debris will be cleaned up today, and she will remind housekeeping to check behind furniture when cleaning

At time of inspection, the facility does not have records of the gas-fired equipment inspections on file. POC: PIC states they are aware of the requirement and are in the process of finding a company to come out to perform the inspections of all gas-fired equipment

At time of inspection, observed stained pillowcases and stained pillow protectors in rooms inspected. Also observed a hole in the mattress pad in Room 124. COS: PIC removed the stained/torn items and will have housekeeping replace with clean items. PIC states she will remind housekeeping to thoroughly inspect all linens

At time of inspection, observed worn/peeling furniture (tables & cabinets) in rooms inspected. PIC states new furniture is expected to be ordered soon - they are determining which pieces need to be replaced

At time of inspection, observed a stained pillow protector in Room 110 and observed a stained pillow protector in Room 122. PIC made note and stated she will have housekeeping replace items
