4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed in use white colored cutting board now brown in color. Code: Equipment food-contact surfaces and utensils shall be clean to sight and touch. CA: if used with time/temperature control for safety food, equipment food-contact surfaces and utensils shall be cleaned at least every 4 hours throughout the day.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed knives stored in the crevice between the surfaces of the steam table and cutting board attached to steam table. code: During pauses dispensing utensils for TCS foods shall be stored either: in the food with their handles above the top of the food and the container; on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a); in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; or in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi).

Observed numerous deep grooves in various cutting boards. Code requires: (b) Cutting Surfaces. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized or discarded if they cannot be resurfaced.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed rusted shelves. Code requires nonfood-contact surfaces of equipment to be kept free of an accumulation of dust, dirt, food residue, and other debris.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(2)(l) - toilet rooms, enclosed (c)
Self-closer missing on water closet door. Observed restroom door open. CA: Keep restroom doors closed except during cleaning and maintenance operations. Code: a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(i) - light bulbs, protective shielding (c)
Light shield missing in Freezer. Code requires that light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food.
