Observed broken toilet lid in lower bathroom. Observed cardboard being used as a temporary cover. RCA: Toilet shall be installed per plumbing code with all surfaces being non-absorbent and easily cleanable.

Observed employee items (purses) stored on prep table upon entry into the facility. Prep table was not in active use. CA: PIC moved purses to hooks provided for employee storage.
