Observed a food employee touch hair with bare hands and proceeded to don gloves in order to prepare a to-go container. Also observed another food employee touched the back of pants and proceed to handle a clean utensils without removing glove to wash his hands. Advised PIC food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food and clean utensils and before donning gloves to initiate a task that involves working with food. COS: Both food employees removed gloves and proceeded to wash hands properly at the designated handwash sink. Food employee discarded To-go plate.

Observed multiple TCS foods in the walk in cooler (sour cream, diced tomato, beef soup, pork soup, salsa, raw shell eggs, milk, raw chicken, shredded cheese, sliced cheese, cooked chicken, Mexi rice) cold holding in the temperature danger zone above 41 degrees F. Advised PIC except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS: PIC discarded all TCS items listed.

Upon arrival, observed food employee fill a SaniBucket at the front designated handwash sink. Advised PIC a handwashing sink shall be maintained so that it is accessible at all times for employee use.

Observed food employee wash hands at the 3-compartment sink. Advised PIC Food employees shall clean their hands in a handwashing sink or approved automatic handwashing facility and may not clean their hands in a sink used for food preparation or ware-washing. COS: PIC had food employee cease washing hands at the 3-compartment sink and designated her to the handwash sink.

Observed food contact surfaces of the interior portion microwave stored as clean with severe rusting near the cookline. Also observed food contact surface the interior portion of another microwave stored as clean with old food debris near the storage area. Advised PIC equipment food contact surfaces and utensils shall be cleaned at any time during the operation when contamination may have occurred. Advised PIC to increase cleaning frequency of the interior portion of the microwave near storage area and to replace the microwave near the cookline.

Observed ambient temperature of the walk-in cooler above 41 degrees F. Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. Advised to have walk-in cooler repaired.

Observed no handwashing signage at the main kitchen's handwashing sink. Advised PIC a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. Advised PIC to post proper handwashing signage at back handwash sink.

Observed, throughout the facility, ceiling tiles with staining in the main kitchen and men's bathroom areas. Advised PIC that physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to adequately clean ceiling tiles throughout facility.
