Observed TCS foods (chocolate milk) cold holding in cooler drawer above 41F. Advised PIC all TCS foods shall be maintained at 41F or below. COS: Milk was discarded.

Observed facility unable to provide agreement forms indicating employee's responsibility to report foodborne illnesses/symptoms. Advised to obtain forms and have employee's sign.
