Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed bulk ice machine in need of cleaning black mold-like substance on interior of ice machine. Advised PIC equipment food-contact surfaces and utensils shall be cleaned: At a frequency specified by the manufacturer; or absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold.

Observed multiple bowls with stains in need of cleaning at warewash area. Advised PIC equipment food-contact surfaces and utensils shall be clean to sight and touch.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed multiple scoops stored in multiple bins of dry rice (2). Advised PIC in food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed facility with ceiling tiles in need of cleaning at main kitchen and warewash area. Advised PIC all physical facilities shall be maintained in good repair.
