Mildew observed on ceiling in #122 in bathroom. Work order has been submitted

Observed debris in dresser drawer for rooms: 258, 257, 161, 141. Observed stains on curtains for rooms 161, 217, 141, 255, 257, 262. Advised PIC all furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed stains and debris on mattress and bed linens for rooms: 258, 262, 161, 217, and mole on mattress protector for room 141. Advised PIC washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed garbage container open. Advised PIC all outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored

Observed visible light entry at front-door of facility for rooms: 201, 203, 217, 141, 239, 258, 257, 262, 265, 161, 154, 149. Advised PIC openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed live pest activity in rooms: 127(dead spider in sink), 141 (live insect activity), and 241 (live spider activity on bed). Advised PIC effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

Observed stains on chair for rooms: 105 and 241; Observed residue in fridge for room 125; Observed build-up on stove top for room: 128; Observed stains on door for room 123, 141, 218. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair
