Observed smoke alarms chirping in rooms 224 and 210.

C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

1. Observed stained mattresses, sheets, and/or mattress covers in rooms 223, 224, 314, 215, 210, and 302.
2. Observed hair on pillowcase in room 314.
3. Observed holes on mattress and/or sheets and linens in rooms 224 and 302.

C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed no anti-slip surface in bathtubs of rooms 223, 215, and 210.

C/A: Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observed stained mattresses in rooms 224 and 326. Observed stained sheet in room 224. C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Correct in 72 hours

Observed stained and peeling carpets through out hallways. C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. Correct within 30 calendar days

Observed several flies present around ice machine in lobby (not in use). C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects

Observed dead worm-like insects inside fridge (unplugged) inside room 224. C/A: The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects

Observed single service cups without wrapping in rooms 326, 314, 311, 226, 224, 223. C/A: Single- service items placed in a guestroom or dwelling units shall be commercially pre-wrapped to protect against contamination

Observed several flies present in lobby near ice machine. C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

PIC unable to show pest control documents. C/A: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises

PIC was unable to show gas fired equipment documentation. C/A: Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed rooms 320 and 306 with stained mattress. Observed rooms 306 and 226 with holes in linens. Observed rooms 306, 324, and 226 with stained linens. C/A: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed pink slime build-up inside lobby ice machine. C/A: Ice machines shall be kept clean. COS: Cleaned

Observed rooms 226, 221, 340, and 302 with stained sheets and mattress covers. C/A: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed pink slime build up on ice dispenser on 3 floor and lobby. C/A: Ice machines shall be kept clean

Observed juice dispenser in lobby with green/white mold-like growth around nozzles. C/A: Food shall be in sound condition. COS: PIC cleaned nozzles
