Observed several cold holding violations through out facility. C/A: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41F or below. COS: Discarded. Chobani yogurt cooled to 34F @ 10:22am.

PIC could not provide in a verifiable manner that all food employees are informed of their responsibility to report their illnesses and symptoms.
C/A: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food

15C - nonfood-contact surfaces clean

Observed mold-like growth and accumulations inside door of juice dispenser of self service counter. C/A: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Correct within 72 hours.
