1. Observed washcloths stained and tearing in rooms 130, 331, 221 and 419.
2. Observed gap in window screens in rooms 331 and 402.
3. Observed window latches in disrepair in room 221.
4. Observed broken light fixtures in rooms 130 and 402.
5. Observed soiled refrigerator and microwave in rooms 130 and 402.
6. Observed refrigerator in disrepair in room 130.

C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

1. Observed stained bedding and pillows in rooms 206, 331, 419, and 402.
2. Observed hairs on bedding and/or pillows in rooms 331, 130, 206, 221 & 402
3. Observed stained bedding in rooms 130 and 221.
4. Observed debris on bedding in room 402.

C/A: CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

2. Observed bath-tub ledge in disrepair in room 130.
3. Observed hair in bath-tub in room 221.
4. Observed stain on wall next to toilet in room 221.

C/A: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed no ventilation in bathroom of room 130 and 331.

C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated

Observed unlabeled chemicals and personal items on housekeeping cart and housekeeping laundry room.

C/A: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items

Observed stained sheets and mattress covers in room 129, 139, 219, 331, 428. Observed stain on couch mattress pad in room 211.

CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed no ventilation in the bathrooms in rooms 129, 211, 219, 331, 428. EHS did not observe any sound from the vents and used the tissue test in several of the bathrooms and did not observe any suction.

CA: Toilet, lavatory and bathing facilities shall be mechanically ventilated

PIC was unable to provide any pest control records.

C/A: Maintain pest control records on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises.

POC: Pest control Records for the last 18 months shall be provided to the health authority by Tuesday, May 13th, 2025

PIC was unable to provide any gas fired inspection.

C/A: Obtain and provide current annual gas fired inspection documentation; ensure all gas-fired equipment is inspected at least annually by a qualified licensed contractor or local fire authority.

POC: Provide annual gas fired inspection documentation to health authority by Tuesday, May 13th 2025.

NOTE: the last annual gas fired inspection report on file is from 4/3/23

Observed stained mattress cover in room 429 and 332. Observed stain on sheets in room 301

Observed inspection report posted out of public view, on a wall across from the door leading into the offices in the front lobby area.

C/A: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed no ventilation in the bathrooms in rooms 429, 411, 332, 301, and 214. EHS did not observe any sound from the vents and used the tissue test in several of the bathrooms and did not observe any suction.

C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed pink slime-like substance inside of ice dispensers where the ice comes out, in ice machines on the 1st and 2nd floors

PIC was unable to provide any pest control records.

Observed housekeeping employee cleaning rooms without shoes on. C/A: Employees shall maintain a high degree of personal cleanliness and conform to other good hygienic practices

Observed the Inspection report from 7/16/2021 being displayed in the main lobby area.

C/A: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times

C/A: Self-service ice-storage machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

PIC and staff could only provide 3 months of pest control records for facility. C/A: Maintain pest control records on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises
