Observed no paper towel at the hand sink located in the main kitchen. C/A Hand Drying Provision. Each handwashing sink or group of adjacent handwashing sinks shall be provided with:

COS Manager refilled the paper towel holder
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed inverted dishes below the grill station with what it seemed to be dried blood and grease splatter on them. After dishes were removed blood was still noticed where they were stored. C/A Equipment food-contact surfaces and utensils shall be cleaned:
(i) Before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry. It does not apply if the food-contact surface or utensil is in contact with a succession of different types of raw meat and raw poultry each requiring a higher cooking temperature as specified under DPH Rule 511-6-1.04(5)(a) than the previous type such as preparing raw pork followed by cutting raw poultry on the same cutting board; P
(ii) Each time there is a change from working with raw foods to working with ready-to-eat foods; P
(iii) Between uses with raw fruits and vegetables and with Time/Temperature Control for safety food; P
(iv) Before using or storing a food temperature measuring device; P and
(v) At any time during the operation when contamination may have occurred. P

Except as specified in paragraph 3 of this subsection, if used with time/temperature control for safety food, equipment food-contact surfaces and utensils shall be cleaned at least every 4 hours throughout the day. P

Surfaces of utensils and equipment contacting time/temperature control for safety food may be cleaned less frequently than every 4 hours if: COS Manager removed the dishes and placed them in the sink to be washed and sanitized the area where they were stored

Observed gnats and flies flying around in the pantry and in the office area. Manager stated the facility was inspected and treated by ecolab on 7/9/2025 yet the problem persists. C/A Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

Eliminating harborage conditions. Update: EHS PERFORMED AN INFORMAL VISIT ON 7/24/2025 TO DETERMOINE IF THE PROBLEM PERSISTS. MANAGER ON DUTY JAMIRIA STATED THAT THE FACILITY IS HAVING A CONTRACT WITH ECOLAB AND THEY HAVE THEM SCHEDULED TO PAY A VISIT TO THEM ON 7/24 EVENING OR 7/25 MORNING. EHS ADIVISED THE MANAGER TO SUBMIT THE INVOICE TO EHS REGARDING THE PROPOSED TREATMENT
14B - utensils, equipment and linens: properly stored, dried, handled

Observed ready to use fruit dishes at the front service area that were stored unprotected and uncovered or inverted. C/A Except as specified in subsection 2(i) below, tableware that is preset shall be protected from contamination by being wrapped, covered, or inverted. COS Manager Inverted the dishes
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed waffle iron that was used yesterday with heavy batter splatter on the shelf ready to use in the main kitchen. C/A Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. COS Manager had 3 days to correct the violation and email pictures to EHS
