Observed wet wiping cloth stored on self-serve table in front of coffee items in dining area. CA: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). PIC had employee to store wet wiping cloth in solution in between uses.

Observed visible large stains on two ceiling tiles in dry storage area. CA: Ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will replace two ceiling tiles in dry storage area.

Observed raw hamburger patties being stored above precooked food (French Toast, and breakfast bagels). CA: Food shall be protected from cross contamination by: Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and Cooked ready-to-eat food. COS PIC rearranged the shelf that the food was being stored on.

Observed black build up in the ice machine at the front of the food service facility. CA: Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: In equipment such as ice bins. COS PIC had employee to clean the ice machine.

Observed several cutting boards that were not clean to sight and needing to be replaced. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC will order new cutting boards.

Observed one employee with no beard restraint on while he was preparing food items. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS PIC had employee to place a beard restraint on.

Observed no hot water at the handwash sink in the facility. CA: The water source and system shall be of sufficient capacity to meet the peak water demands of the food service establishment. PIC contacted maintenance to come and look to see why the hot water was not working.

Observed food service permit not posted for patients to visibly see. CA: Post the permit in a location in the food service establishment that is conspicuous to consumers. COS PIC moved food service permit for patients to visibly see it.

Observed a wet mop not hung to properly dry. CA: After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. COS PIC placed dirty mop head in the dirty container outside of facility.

Observed build up of grease and food debris accumulation on oven under vent hood in kitchen area. PIC will have oven under vent hood in kitchen area cleaned.
