Observed dry storage items and egg rolls in brown chicken box not properly labeled. CA: Label all food items throughout kitchen.
12A - contamination prevented during food preparation, storage, display

Observed egg rolls stored in a brown raw chicken packaging box as well as food items stored on floor in walk-in freezer. CA: Do not reuse chicken boxes for storage and store items 6 inches off ground.

Observed employee preparing food without hair restraint. CA: Ensure hair restraints are worn when working with food.
