13-202(m):Safety rules must contain minimum 12 required statements. Observed rules sign missing statements. Provided CPO required sign list.

13-202(i): Pool facility shall have a departmentally approved EAP. Observed EAP not posted at pool, and missing CDC contamination procedures. Provided CPO with link to CDC contamination procedures.

13-187(c): Operating permit shall be framed and displayed where it is easily readable. Observed permit kept in binder.

Pool shall be supplied with a readily available first aid kit. Observed first aid kit in disrepair.

13-198(b): Ventilation of fresh make up air must be provided in equipment room. Observed ventilation system not functional.

13-190(a): equipment shall be operated to maintain adequate turnover. Observation: Flow meter reads 60gpm. According to gallons of 30,000, flow meter should read at a minimum of 83.3.

13-202(m): Rules must contain at a minimum the 12 statements. Observation: Rules is missing: No animals other than sight-seeing dogs are allowed, no solo bathing, no food or drink within 5 ft of the pool, no spitting, spouting or boistrous play, no running.

Turnover rate must be at least 94 according to calculations of the number of turnovers for 34000 gallons. Observed 71ppm on flowmeter.

13-189(c)(6) At all times when a pool is in use, the water shall be of such clarity that the main drains on the bottom of the pool...are clearly visible. Observed the water as cloudy.

13-202(g) An operable hard-wired, weatherproof telephone, with direct 911 access or capability installed in a conspicuous location shall be readily available.... 13-202(h)The telephone number of the pool operator, manager, and health department. Phone needs servicing. Also missing one of the emergency numbers
